Foster, kinship, and adoptive medically fragile children may be considered for admission to the Lilburn Transition Care Home.

Dream House accepts referrals from various sources, including but not limited to, public and private agencies, physicians, hospitals, and nursing homes.

Up to six medically fragile children may temporarily reside in the home, at any given time, for individual periods 30 days up to one year.

On average, children in our care stay 90 days until Dream House staff transition them into safe and loving homes uniquely prepared to help them thrive.

Please note: to protect the children under our care and supervision, all visits to the Lilburn Transition Care Home must be scheduled with the Dream House Corporate office. Please call 770-717-7410.

Yes, there are medically fragile children in the house today!!!!

The Dream House Family for Keeps™ – Transition Care Program provides temporary, family-centered home care for medically fragile children while their own family members and homes are prepared to care for them.

This facility has been retrofitted to meet the needs of any medically fragile child age two weeks to 11 years. Learn who we serve.

Donations to support direct daily care costs for each child living in the Lilburn Transition Care Home.

Training for anyone involved in the life care of a medically fragile child. Dream House is now accepting registrations for upcoming classes.

Recruits – Dream House is actively searching for individuals and families interested in becoming foster parents or respite care providers. Contact us today for more details.

Dream House relies on financial contributions to cover daily direct care costs for children residing in the Lilburn Transition Care Home.

For each child in the Lilburn Transition Care Home, we need 100 people to donate $30 each month.

At any given time, we may have up to six medically fragile children in the Home.

On average, children in our care stay 90 days until Dream House staff transition them into safe and loving homes uniquely prepared to help them thrive.

Dream House needs to raise $3,000 per child per month so we may provide the food, clothing, health care and home environment these kids need to stabilize their health conditions.
